Aim One: Team for Reducing Cardiovascular Complications in Dialysis Patients

Nephrologists: Responsible for assessing whether patients need cardiovascular examinations.
Cardiologists: Responsible for evaluating test results and developing treatment strategies.
Cardiac Catheterization Technicians: Assist in performing cardiac catheterization and completing treatments.
Case Managers (Nephrology Department): Responsible for tracking cases and advising patients to return for regular follow-ups.

Aim Two: Team for Reducing Infection Risks in Dialysis Patients

Nephrologists and Specialist Nurses (Department of Internal Medicine, Nephrology): Arrange and assist in issuing laboratory tests, explain reports, and address patient inquiries.
Infection Control Physicians: Responsible for designing and promoting the plan.
Infection Control Nurses: Perform nasal swabs and provide training for healthcare staff in the dialysis unit.
Dialysis Nurses: Assist with patient education and advocacy.
Medical Laboratory Department: Assist with bacterial species identification and strain preservation.
Faculty of Microbiology and Immunology (Taipei Medical University): Assist with bacterial strain typing and antibiotic resistance gene analysis.

Multidisciplinary Team Composition:

Internal Medicine, Nephrology Department: 5 physicians
Cardiac Testing Unit (Cardiac Ultrasound, Cardiac Catheterization, Nuclear Medicine Testing): 13 members (7 physicians, 2 catheterization nurses, 2 radiology technologists, 2 case managers)
Infection Control Unit: 9 members (3 physicians, 4 nurses, 1 medical technologist, 1 secretary)
Medical Laboratory Department: 4 members
Specialist Nurse (Nephrology Case Manager): 1 member
